This policy explains how ValtaGuard ("we," "us") handles information collected through this website. It covers website inquiries only. Information we handle while providing paid services is governed by the Service Agreement and Business Associate Agreement signed with each practice.
Do not send patient information
This website and its forms are not designed to receive Protected Health Information. Please do not submit patient names, records, images, dates of birth, or any other Protected Health Information through this site or by unencrypted email. If Protected Health Information is received through the website by mistake, we will delete it and will not use it.
Information we collect
When you use a form, we collect what you enter: your name, office name, email, phone number, workstation count, practice software, and your message. We may also collect basic technical information such as browser type and pages visited through standard server logs.
